outline the dissertation methodology and design a questionnaire for the dissertation

Please check the attachment, there are some feedback from my tutor about my dissertation poprosal. And the methodology part is so pool, please following the feedback from my teacher help me to revise the methodology part of the poprosal, and give me an outline about what should I write in the dissertation.
