Write a petition to University of Oregon with a personal statement on what is my plan to enhance my study if I got back to UfO

So, I got disqualified from University of Oregon because of the low GPA, four terms in a row. Now I have been studying in Lane Community College for 1 year and planning on transfer back to UfO on winter term. You are gong to write a petition, there will be my personal statement on what proof can i provide that if reinstated, i will be academically successful? What factors have changed since i last attended? What is my plan to enhance my study if I got back to UfO?
